The All-on-4 Treatment Day: What to Expect

Pre-treatment

Before your implant surgery, you’ll have had a consultation and a CBCT (3D) scan. The CBCT shows us your bone height, density, and the position of your sinuses and nerves — essential for safe, precise planning. We design the exact implant placement on the CBCT before any clinical work.

The morning of surgery

You’ll arrive at the practice. If you’re anxious, we offer local anaesthetic with nitrous oxide (gas and air) or IV sedation — your choice, discussed at consultation. Sedation patients need a driver.

The procedure (typically 2–3 hours)

• We use local anaesthetic so you’re comfortable; most patients feel pressure and vibration rather than pain

Your bone may have shrunk slightly, which is why we always take a CBCT scan to measure bone height and density. What if I don’t have enough bone for All-on-4? We assess this with a CBCT scan. If bone height is borderline, the two tilted implants at the back of an All-on-4 can often reach into available bone without needing a graft. If grafting is genuinely needed, we’ll quote it separately and discuss the timeline and cost with you upfront.

Am I a candidate for “same-day teeth”? Possibly. Same-day teeth (temporary bridge fitted on surgery day) are possible in suitable All-on-4 cases. A few patients (those with very poor bone or recent extractions with active infection) may need to wait a week or two for swelling to settle before the temporary teeth are fitted.
